Read to the team member
Your team has engaged a third-party consultant to conduct a team collaboration diagnostic. The purpose is to understand how your team collaborates — what's working, where it breaks down, and what changes would have the most impact.
This is not an evaluation of you. It's a conversation about how the team functions, from your perspective.
What we'll do: I'll start with a few open-ended questions, then we'll move into ~250 yes-or-no questions across six domains of collaboration: culture, leadership, strategy, execution, learning, and innovation. Most people take about 60 minutes.
Your responses: For each question you can say yes, no, unsure, or skip. Only answer yes if you're fully confident the answer is yes — no and unsure are both perfectly valid responses. Skip if the question doesn't apply or you'd rather not answer. You can also flag any question as high-impact — meaning "this question is especially important to me, regardless of my answer." It helps us identify where to focus.
The more you share, the better. The yes/no answers give us structure, but the real value is the conversation around them. If something makes you pause or you want to qualify — please share. Feel free to take a break at any point.
Confidentiality: Your individual responses will never be shared — not with your boss, not with your team leader. I'm a third-party consultant and I'm not obligated to share any personal information you share with me. Only aggregated team data is presented.
Recording: This interview will be recorded and transcribed so I can capture all the details. The recording is kept confidential.
How results are used: After all interviews, the findings will be discussed as a team in a facilitated workshop and with your leader to support their growth and development.
